Jung Wellbeing Informed Consent Form
Acupuncture and Remedial Massage treatments are generally effective and safe; however, like any treatment, they come with benefits and risks. The purpose of this form is to outline your rights and how we approach collaborative decision-making and informed consent between practitioner and patient.
Practitioners in this practice will discuss your condition and treatment options with you, ensuring you are well-informed and able to make decisions regarding your treatment. You may choose to consent or refuse any form of treatment for any reason, including religious or personal grounds. You have the right to withdraw your consent at any time.
Practitioners reserve the right to refuse service with reasonable and non-discriminatory justifications.
Personal Questions: Your practitioner may ask personal questions related to your condition and how it affects your daily life. The more information you provide, the more effective the treatment can be. You have the choice of what information to share, and if you are uncomfortable with specific questions, please inform the practitioner.
Physical Contact: During the examination, assessment, and treatment, your practitioner may require physical contact, which will be done with your express consent. You can withdraw consent at any time, and all physical contact will cease immediately. Please let your practitioner know if you ever feel uncomfortable.
Risks Related to Treatment: As with all treatments, there are risks and benefits. Your practitioner will discuss foreseeable risks before administering treatment. In some cases, you may be asked to read and sign additional consent forms to ensure you fully understand the risks involved. You can withdraw consent at any time, even after signing a consent form.
Children and Minors: Treatment of a minor (under 18 years of age) requires consent from a custodial parent.
Substituted Consent: If a person cannot understand the risks and benefits of treatment, consent may be provided by another legally authorized person. Evidence of legal authorization is required in such cases.
Jung Wellbeing Privacy Policy
We may collect, use, and disclose your Personal Health Information and Health Record for various purposes, including communication with other healthcare professionals, follow-up/reminder calls, discussions with third-party insurers, health insurance procedures, consultations with you, and legal disclosure as required by a court of law. If you have concerns or wish to restrict access to your information, please discuss this with your practitioner. This practice adheres to National Privacy Principles (www.privacy.gov.au) and has a written policy available for your review.
Jung Wellbeing Practice Policy
At Jung Wellbeing, our goal is to provide you with exceptional, prompt, and professional acupuncture and Remedial Massage care. Based on our experience, we've identified key areas to ensure you benefit the most from our services.
Mobile Phone: To respect others, please turn off your mobile phone during your visit.
Healing: Healing and correction take time, and not everyone heals at the same rate. If you ever feel you're not responding as expected during your care, please discuss it with your practitioner. Our aim is to help you get the most from your care at Jung Wellbeing.
Financial Arrangements: Patient fees are due at the time of service. We accept cash and offer EFTPOS facilities in the clinic room. Private health funds are accepted for eligible clients, and a receipt with provider details will be emailed post-treatment for submission to your private health fund.
Appointment Scheduling/Missed Appointments: To achieve the best outcome and save time, please schedule appointments in advance. Missed appointments can set back your recovery, and we ask that you keep all scheduled appointments whenever possible. Changes, missed appointments, or cancellations made within 24 hours of the scheduled appointment will incur a cancellation fee, which is not covered by compensable bodies and must be paid by the patient.
Initial Consultation: All new patients receiving Acupuncture treatments must book an Initial Acupuncture session. Patients who haven't received treatment at our clinic for more than 12 months must also book an Initial Acupuncture.
Cancellation Policy: Please provide a minimum of 24 hours' notice for appointment cancellations or changes. This is essential because your session time is reserved exclusively for you, and we cannot allocate it to another individual without ample notice. If you are unable to attend, you may substitute a friend or family member*. However, not showing up or making changes to an appointment with less than 24 hours' notice will result in a cancellation fee equal to 50% of the appointment cost. Arriving late for your appointment may prevent an extension of the session to ensure punctuality for the next appointment.
* If the person substituting for you receives a treatment with a lower cost than your scheduled session, the difference will not be refunded. Conversely, if the substituted person receives a more expensive treatment, they will be responsible for covering the price difference.
Refund Policy: Please choose products carefully as we do not offer refunds for services due to a change of mind.
Deposits: Appointments canceled or changed with more than 24 hours' notice will result in a full deposit refund. Not showing up or making changes to a scheduled appointment with less than 24 hours' notice will not result in a deposit refund. This helps cover clinic and practitioner costs.
Prepaid Package Purchases: Prepaid Package Purchases are non-refundable.
Hydration: Staying well-hydrated is vital for your health, so we provide filtered drinking water. The water is prepared in the waiting area.
Young Children: For your safety during treatment, please ensure young children are safely supervised.
